Intake forms on WhatsApp: collecting pre-visit information in conversation, what to ask, what to keep off the chat, and how an AI agent runs the intake after booking

The intake PDF has a completion rate most businesses would rather not know. The same questions asked one at a time in WhatsApp, right after the booking, get answered because answering is one tap. The art is asking few questions, asking them at the right moment, and keeping the sensitive ones off the chat.

Quick answer

Right after a booking is confirmed, ask three to six intake questions in conversation, one at a time (reason for visit, relevant history in general terms, preferences, how they heard about you, anything you need to prepare), confirm the answers back, and store them against the appointment. Use a secure form link, not the chat, for anything sensitive or regulated (medical history, ID numbers, financial details). An AI agent runs the intake automatically after each booking and attaches the answers to the lead; ReplyKit does this when you add the questions to the agent's instructions.

Keep the chat intake to five or six questions. The data rules for the sensitive column are in GDPR and AI agents.

When to ask

  1. Immediately after the booking confirmation, while the customer is in the chat: "To get ready for you, three quick questions."
  2. One question per message; wait for the answer.
  3. If they stop mid-way, the reminder the day before repeats the unanswered questions once.
  4. Confirm the answers back in one message so the customer can correct them.
  5. For forms that must be completed, send the link with the confirmation and again in the reminder, and have the agent answer "what do I need to fill in" from your note.

How the agent runs it

Add the questions to the agent's instructions as a post-booking step ("After confirming a booking, ask these questions one at a time and record the answers"). The answers are stored in the lead's record and visible when you open the appointment. The agent also answers the customer's own pre-visit questions (parking, what to wear, how long) from your knowledge, which cuts the day-before messages. For medical and regulated practices, the agent collects only the general column and sends the form link for the rest; the clinic setup is in clinics and clinic templates.

Use the answers

An appointment that starts with "you mentioned the pain is worse in the morning" or "you asked for the quieter room" is the whole point. Review intake answers in the morning for the day's appointments. Over a month, the "how did you hear about us" answers tell you which marketing works, which feeds tracking leads from ads.
